Medicare, the federal health insurance program for the elderly and disabled, pays for home health services from approved providers. Home health services include medication management, meal preparation, self-care and assistance with other activities of daily living, housekeeping, transportation and companionship. Services are provided by registered nurses, licensed nurse practitioners, home health aids, certified nurse aids and non-licensed home care aids. Payments for home health services are made under Medicare Part A (inpatient services) and Part B (outpatient services).

Contact your state's licensing agency for home health services to determine what licensing requirements must be met to operate as a home health agency. The Centers for Medicare and Medicaid Services offers a search mechanism for locating what agency must be contacted in your state (see Resource 1, below). Ensure that all proper licensing requirements are met in your state prior to applying to be a Medicare-approved provider.

Obtain a National Provider Identifier (NPI) number by calling the National Plan & Provider Enumeration System, at (800) 465-3203. All providers that accept Medicare, Medicaid and private health plans must have an NPI number.

Complete Form CMS-855A from Medicare (this form is also known as "Medicare Enrollment Application -- Institutional Providers"). You can obtain this form from the Medicare website (see Resource 2). This form requires a state licensing number and an NPI.

Complete supporting forms. These forms are also available from the Medicare website (Resource 2). You will need to complete form CMS-588, a Medicare authorization for electronic funds transfers; and form CMS-460, an agreement to become a Part B participating provider and an electronic data interchange agreement.

Submit all forms to the designated places that are listed on the forms. Typically, it will take 60 days to process the application and supporting forms.
